It is a sequel to the 2009 film Sherlock Holmes, based on the character of the same name created by Sir Arthur Conan Doyle. The screenplay is written by Kieran Mulroney and Michele Mulroney. Robert Downey, Jr. and Jude Law reprise their roles as Sherlock Holmes and Dr. John Watson, respectively, as the duo join forces to outwit and bring down their most cunning adversary, Professor Moriarty, played by Jared Harris. The film is specifically influenced by Conan Doyle's work The Final Problem, but it is an independent story rather than a strict adaptation.
Sherlock Holmes (Robert Downey, Jr.) tracks Irene Adler (Rachel McAdams), his old lover and a master criminal, who is acting as a courier for Professor Moriarty (Jared Harris). Moriarty is the criminal mastermind, Holmes' intellectual equal, behind the criminal activities in the first Sherlock Holmes film in this series. After slipping Holmes, Adler delivers a package to a doctor at an auction. During this very quick exchange, mention is made of a note from a Moriarity conspirator for the conspirator's sister. Holmes reappears. As Adler's package is revealed to be a bomb, Holmes and Adler spar and Holmes steals the note from Adler. Although he defuses the bomb, Holmes fails to prevent the doctor's assassination. Adler reports to Moriarty by meeting him at a public restaurant. Moriarty then releases Adler from his employ, saying that her feelings for Holmes have compromised her usefulness and led to the theft of the letter. Adler starts to leave, but then stumbles and falls as Moriarty had poisoned her tea.
The next day, Dr. Watson (Jude Law) is about to be married and arrives at Holmes's home, ready to go out for a stag party he believes best man Holmes has arranged. Holmes shows himself to be upset by Watson's marriage to Mary (Kelly Reilly). Holmes has become dissolute, living on caffeine, tobacco, and coca leaves and drinking embalming fluid. He also reveals his experiments in urban camouflage, hiding in plain sight. Holmes reveals that he has investigating a seemingly unrelated series of crimes, believing them all connected to Professor Moriarty. Holmes believes Moriarty is using his connections and wealth to arrange 'anarchist' bombings in France and Germany, creating tensions between both countries.
Holmes introduces Watson to his brother, Mycroft (Stephen Fry), who has an undefined, high level role with the British government. Mycroft informs Holmes that he must attend a peace summit in Switzerland soon due to the recent bombings. As the three of them reach the party, Watson realizes Holmes has failed to invite any of the guests Watson had expected. At the 'party', it becomes apparent that Holmes has brought Watson there to find another intended target of Moriarty's, a fortune telling gypsy named Sim (Noomi Rapace). Holmes shares the stolen letter with Sim, tying her to Moriarty through her brother, Rene. Holmes also informs Sim that Moriarty has sent an assassin to kill her. Holmes attacks the assassin, eventually escaping with Watson and arriving the next day still drunk, late, and in scandalous dress for Watson's wedding. Following the ceremony, Holmes is told to meet Moriarty at the University. Holmes meets with Moriarty, who warns Holmes that if he persists in investigating him, Watson and his wife will be targeted, and that Holmes' lover, Adler, had already been killed.
The next day, Holmes stows away, disguised as a woman, on the train taking Dr. Watson and Mary to their honeymoon destination. While in transit, several dozen of Moriarty's men, disguised as attendants and British soldiers, attack Watson and Mary. Holmes throws Mary off of the train into a deep river, where Mycroft picks her up and takes her to his home. Watson, though furious, agrees to travel with him to Paris to confront Moriarty - who is traveling on a lecture tour - and find the gypsy whom Holmes had saved earlier. When the duo arrive in France, Holmes locates Sim, as she is the only clue to Moriarty's plans. He tells Sim that Moriarty targeted her due to her brother Rene's work with him, and she was a loose end who would be killed. They deduce Rene's location in Paris from the note that Holmes stole from Adler prior to her murder, and arrive there to find a radical anarchist group that Rene and Sim used to work for. The anarchists had been forced to plant bombs on Moriarty's behalf, in part because Moriarty has kidnapped the leader's wife and children. The leader informs them that the last bomb had been planted that night, and that his family would finally be free if he tied up loose ends. He then commits suicide.
Holmes, Watson, and Sim follow the clues that Holmes deduced believing that Moriarty would strike in the Opera and arrive there that night. Holmes realizes too late that Moriarty has deceived him when he discovers that Moriarty is present. Instead, a hotel where a major business meeting was occurring is blown up. As Holmes looks over the bodies, he realizes the bomb was a cover for a gunman of Moriarty's, Colonel Sebastian Moran (Paul Anderson). Colonel Moran was previously the finest sharpshooter in the British Army and had almost supernatural marksmanship. Just prior to the bomb exploding, Moran shot a German businessman at the party, ensuring his death. Holmes notes that the businessman had previously owned a massive arms company, which had recently had a majority of its stock purchased by an unknown investor. Holmes, believing the investor is Moriarty, crosses the border with Watson, Sim, and her gypsies, investigating one of the company's factories in Germany. Holmes and Watson separate, but Holmes is captured by Colonel Moran and interrogated by Moriarty. Moriarty tortures Holmes by impaling him through the shoulder on a meat hook, lifting him off of his feet, and dancing with him as he is suspended. Moriarty reveals he owns shares in companies across Europe in cotton, guns and other goods, and plans to start a war that will create a large demand for them and make him a fortune. Watson, attempting to save Holmes, is nearly killed by Colonel Moran, but manages to escape by collapsing Moran's sniper nest in a large, lighted tower. The tower's collapse nearly kills both Holmes and Moriarty. The duo rendezvous with Sim and her gypsies, pursued hotly by German Army soldiers and Colonel Moran. Most of the gypsies are killed by artillery and mortar fire, as are the German soldiers. Moran is shot, but not killed, by Watson, as the gypsies, Holmes, and Watson attempt to board a moving train to escape. Only Sim, Holmes, Watson, and one other gypsy make it onto the train. Holmes, suffering from multiple shrapnel injuries, stops breathing on board the train, but is revived when Watson administers a shot of adrenaline given to him by Holmes as a wedding gift. Holmes surmises Moriarty's next target is the peace summit in Switzerland, where Moriarty will create an incident between world leaders to spark war.
Holmes, Watson, and Sim are given invitations to the summit by Holmes' brother Mycroft. At the summit, Holmes deduces that a set of twins working for Moriarty in Germany were not actually twins. They were the result of an experiment, by the doctor killed at the auction, to give a man the face of another. Watson and Sim begin investigating the guests, realizing then that one is Rene, disguised as an ambassador, and acting as an assassin. Meanwhile, as they search, Holmes invites Moriarty, who is also present at the summit, to a game of chess on a balcony over a waterfall. Watson and Sim are able to stop Rene; however, he is surreptitiously killed by Colonel Moran. This means that Holmes cannot prove that Moriarty was behind the assassination attempt, as Rene was the only loose end that Moriarty had left. Following this, Holmes reveals to Moriarty that in Germany, he stole the account book tracing all of Moriarty's assets, and replaced it with a duplicate. The account book was the only remaining piece of evidence linking him to his deeds. This book had been sent to Mary in London (apparently by a gypsy courier), who deciphered it using a key discovered by Holmes while visiting Moriarty's university office. Mary passes this information to Inspector Lestrade (Eddie Marsan) who begins seizing dozens of boxes in Moriarty's office. Holmes then checkmates Moriarty ending their game.
Holmes, standing at the edge of the balcony, asks Moriarty to light his pipe. During this time, each man prepares to do battle with the other, and mentally script out the fight in their respective minds. Both realize that Holmes will eventually lose due to his shoulder injury. With no other option, Holmes blows soot from his pipe in Moriarty's eyes, distracting him. Holmes then grabs Moriarty, pulling him in close, and Holmes sits on the balcony ledge while bracing his foot on a nearby table. As Watson enters the balcony from the ballroom, Holmes gives him a brief look then pushes off the table, throwing Holmes and Moriarty backwards over the edge. The two fall hundreds of feet to their apparent deaths at the base of the waterfall. At Holmes' funeral, it is revealed that the bodies of both men were never found. Some time later, Watson and Mary prepare to go on a honeymoon to replace the interrupted one, while Watson finishes writing of his last case with Holmes. A delivery of a tiny oxygen producing apparatus that Watson had seen Holmes borrow from Mycroft alerts Watson that Holmes may have survived. Watson runs from the room to question Mary about the delivery. Holmes reveals himself to the audience, having concealed himself in Watson's room using his urban camouflage suit, before rushing to the typewriter and adding a "?" after the words "The End", in the writing done by Watson.
